Sassafras oil was used by Native Americans for many centuries, primarily for infections and gastrointestinal problems. In ancient herbal medicine, sassafras was used in the treatment of high blood pressure, rheumatism, arthritis, gout, menstrual and kidney problems, and for skin complaints.

Sassafras was one of the first and largest exports from the New World to Europe as a beverage and medicine. Commercially, this pleasant, volatile oil was used as a flavoring agent in root beer and other soft drinks. Physicians in the late 19th and early 20th centuries considered sassafras a useful diaphoretic (a substance that causes sweating) and diuretic plant for relieving rheumatism and fevers, as well as treatment of urinary tract infections.

Sassafras oil contains up to 85% of the terpenoid known as safrole. There are two important sassafras oils of commerce: Brazilian sassafras oil, obtained from the trunkwood of Ocotea pretiosa, and Chinese sassafras oil from Cinnamomum camphora. Both contain 80 percent or more of safrole. More than half the sassafras oil currently entering world trade originates from the People's Republic of China.

Sassafras is often combined with other herbs to purify the blood and stimulate the liver. Applied externally, it contains antiseptic properties making it useful in the treatment of skin wounds, sores and rashes. It has also been used as a massage oil to penetrate the skin, strengthen muscles and sinew, stimulate the circulation of blood and lymph, and assist with nervous system coordination.
